Estimated Cost Savings millions if Primary Care Related ER Visits in Harris County Took Place In An Outpatient Setting Instead 2009

This program provides an after-hours pediatric clinic at three FQHCs in Harris County as an alternative to emergency departments. These clinics are accessible to pediatric patients of low income families. A research team from the University of Texas School of Public Health has conducted an evaluation of this program and concluded that it resulted in a reduction in emergency care utilization in the targeted population and in lower overall health care cost.

Improving Eligibility Screening through Medicaider - Complete

Uninsured 2011This project offers member community health centers access to a web-based eligibility screening system that more effectively and accurately screens uninsured patients for coverage and benefits. It streamlines the screening process and increases the number of Harris County residents receiving benefits. As of November 2012, the program had screened 12,150 uninsured residents and identified 2,930 Medicaid eligible residents.

ConnectingThis project brings together four HCHA-member community health centers and connects them with major health systems in Houston through health information exchange technology. The initiative will also improve operating efficiencies through clinical and administrative benchmarking, best practices and quality improvements. Shared data will give safety net providers increased access to patient histories and change referral patterns (electronic vs. phone referrals). The project will allow participating health centers to: (1) collaborate to identify improvements; (2) enhance capacity and physician productivity through use of a shared MBA fellow; and (3) ensure sustainability through access to a community-wide patient health portal for sharing data.

Charity Care Clinic’s Shared Quality Program

In this project, participating clinics will share one quality program director. The project will consist of three phases: 1) Assessment of current clinical capabilities; 2) Provision of the necessary tools and training for volunteers and staff to track and analyze data; and 3) Implementation of the shared practice management system. The shared quality program ultimately will result in more consistency in reporting while preventing redundancies in the staffing infrastructure as well as aid in comparing the needs of the community in relation to the services offered.
